As we can see here on these pictures, the challenge is not (as I often hear) in the posterior area to preserve esthetics… the challenge is to create enough soft tissue thickness to prevent soft tissue collapse towards the the direction of the implant resulting in a thinner tissue contour…

PINK IMPLANT PROSTHETIC

📆Case Done in 2018

Previous implant crown showing detrimental effect of cemented retained strategy onto bone stability, tissue stability leading to esthetic compromise.
In this case, surgical approach seems unrealistic (at least in my hands).
So we aimed for prosthetic esthetic restablishment by changing the shape of both central incisors and correcting the pink deficiency.
21: Felspathic ceramic chips by the talented @mosconisebastien
11: Screw retained implant crown with pink porcelaine extension to compensate for Distal papilla loss. 
Ti-base with correction of angulation to compensate for Buccal screw axis of the implant.

Cleaning and maintenance are the key here and patient is monitored every year.
This option is to considered only with very cooperative patient - and she is doing great. In the first two years, Torque insertion was provisionnal (20Ncm) to allow me to retrieve easily the crow and check for plaque control.

In these type of cases, Prosthetic work and patient cleaning are more important than dentist work. Full credit to @mosconisebastien for his amazing labwork and to the patient for this.

1years and 3 years FU control showed great stability. Bone seems to grow Back

-Desinfection of the Provisional prosthesis
-Crown delivery: if the design is right, no anesthesia is needed.
